Capitation payment unit and dissatisfaction with the health care system: spatial characterization of Medellin

Abstract

This study uses information from the Quality of Life Survey to segment the population of Medellín according to the characteristics that determine satisfaction with the health system. The segmentation, using the k-prototypes algorithm, considers possible changes in space according to the communes and corregimientos of the municipality, and changes over time from the years 2011 to 2019. It was obtained that the population can be segmented into two groups, but both are dissatisfied with the health system. In addition, the socioeconomic improvement, the high values of risk premiums of the Capitation Payment Unit delivered to the Health Promoting Entities and the physical availability of health service delivery points do not lead to the satisfaction of the affiliates, and much less to an improvement in the perceived state of health.
